Atopica (cyclosporine) and the equivalent veterinary product Modulis are oral immunomodulator medications used to control atopic dermatitis (chronic allergic skin disease) in dogs and cats. By selectively dialing down the activity of T-cells and other immune mediators that drive allergic inflammation, cyclosporine can dramatically reduce itch, redness, and skin flare-ups in patients whose allergies are not adequately controlled by topical therapy or antihistamines alone. Unlike steroids, cyclosporine does not cause increased thirst, increased urination, hunger, panting, or muscle wasting, which makes it a popular long-term option in patients who need steroid-sparing immune modulation.
What Atopica and cyclosporine are used for
The primary indication is canine atopic dermatitis — the chronic, relapsing itchy skin disease of allergic dogs. Cyclosporine is also widely used in cats with allergic dermatitis (eosinophilic granuloma complex, miliary dermatitis, head and neck pruritus), and off-label for immune-mediated diseases such as perianal fistulas, sebaceous adenitis, anal sac fistulas, immune-mediated hemolytic anemia, and chronic enteropathy. Onset of effect typically takes 4–8 weeks, after which the dose can often be reduced to every-other-day or twice-weekly maintenance.
Important considerations
The most common side effects are GI: vomiting and soft stool, which usually improve after the first week and can be reduced by giving the capsule chilled or freezing it briefly, or by giving with a small amount of food. Less common effects include gingival overgrowth and a dull coat. Because cyclosporine modulates the immune system, your veterinarian may recommend periodic infectious-disease screening and bloodwork during long-term use. Avoid live vaccines while on therapy. Give 2 hours before or after meals (Atopica oral solution and capsule absorption is reduced by food). Cyclosporine has many drug interactions — disclose every medication and supplement to your veterinarian.
