Tramadol is an atypical opioid pain medication used in dogs and cats to treat moderate pain, often as part of a multi-modal pain plan combined with NSAIDs (carprofen, meloxicam, Galliprant), gabapentin, or other adjuncts. It works through a combination of weak mu-opioid receptor activity and inhibition of serotonin and norepinephrine reuptake. Recent veterinary research has shown that tramadol's pain-relieving effect in dogs is more limited than once thought because dogs convert it to its active metabolite poorly — for that reason, it is typically used as part of a broader pain protocol rather than as a stand-alone analgesic.

What tramadol is used for

Common veterinary uses include post-surgical pain (especially in combination with an NSAID), chronic osteoarthritis pain in patients who cannot tolerate NSAIDs, cancer-associated pain, and certain neuropathic pain syndromes. In cats, tramadol can produce more reliable analgesia than in dogs because cats produce more of the active metabolite — but it is bitter, hard to disguise in food, and many cats refuse it without compounded flavoring. In some U.S. states, tramadol is a controlled substance and a paper or e-prescription is required.

Important considerations

Common side effects include sedation, mild GI upset, and constipation. Less common but serious effects include seizures (especially at high doses or in patients with a history of seizures) and serotonin syndrome when combined with other serotonergic medications (fluoxetine, trazodone, SAMe, certain MAOIs). Disclose every medication your pet is on before starting tramadol. Avoid in pets with severe liver or kidney disease, and use cautiously in pets with a seizure history. Do not stop abruptly after long-term use. Keep tramadol secured — diversion and accidental ingestion by people are real risks.
